If you are a new client, an intake packet will be sent for you to complete electronically prior to our first session. The following forms are some important disclosures to reference before beginning services.
If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), complete this form to authorize release of psychotherapy information.
